Client’s care assessment, coordination and care plan is conducted by our Registered Nurses. A comprehensive assessment of a client’s health condition in collaboration with the client’s health team helps in developing the right care plan.for the client. We may also help coordinate medical appointments.and ensuring that the individual’s healthcare needs are effectively met.
